Full-time Residential Targeted Case Manager (TCM)

St. John Center is on a mission to help people experiencing homelessness address the barriers to housing and self-sufficiency so they can leave homelessness for good. Our vision is a city where all people experiencing homelessness have the resources and support they need to leave homelessness for good.

St John Center, Sheehan Landing has an opening for a Full-time Residential Targeted Case Manager (TCM), in Kentucky's first Single Site Permanent Supportive Housing Program. Residential TCMs Support formerly homeless people with disabilities to achieve housing stability, utilizing the principles of Housing First, Trauma-Informed and Person-Centered Care, Motivational Interviewing, Strengths-Based Case Management, and Harm Reduction.

Minimum Qualifications:

  • Bachelor’s Degree in a behavioral science
  • Willingness to become a Targeted Case Manager
  • Basic computer literacy including word processing and Google suite
  • Effective communication skills
  • Must have a personal automobile, valid driver’s license, liability insurance, and willingness to transport co-workers and/or clients

Preferred Qualifications:

  • Be an approved Targeted Case Manager with the state of Kentucky
  • 5 years of experience in working with the homeless population
  • Ability and willingness to safely and effectively de-escalate situations, including conducting physical interventions when necessary to maintain safety as directed by the agency

Young Adult Homelessness Resources Coordinator 

The Coalition for the Homeless

The Coalition for the Homeless is hiring a full-time young adult homelessness resources coordinator who will be an integral part of the planning team supporting locally and federally funded agencies in the Louisville/Jefferson County Continuum of Care with an emphasis on ensuring that no young adults need to sleep outdoors. 

Duties include: 

  • Developing strategies to address young adult homelessness based on local needs and best practices
  • Seeking opportunities to increase resources
  • Creating and managing monthly case conferencing and service provision
  • Advocating for policies and funding to address young adult homelessness and creating and tracking the local plan to address young adult homelessness.

Bachelor’s Degree in a related field preferred. Must have the ability to learn basic online platforms with which you may not be familiar, strong interpersonal skills and the ability to develop strong work relationships across sectors. Must have demonstrated project management experience. 

Salary:  $57,000 to $62,000 based on experience
